Job Description

 

Job Title: Inclusion Support Worker

Reports To: Rachel Taylor, Interim Manager

Location: Community-based in Merseyside

Hours: Full-time or part-time (flexible)

Salary: £12.21 per hour, increasing to £12.70 per hour upon successful completion of a 6-month probation period

Job Summary

As an Inclusion Support Worker, you will empower disabled people to be valued as equals in their community, supporting them to build meaningful connections, friendships, and belonging in line with ACT’s vision and mission. You will deliver person-centred, rights-based support, promote independence, and remove barriers to inclusion while upholding our values of Equality, Human Rights, Integrity, Inclusion, and Relationships. Key Responsibilities

  • Support disabled people individually or in groups to develop skills, participate in activities they love, and achieve personal goals in a person-centred, rights-based way, valuing their autonomy and equality.

  • Foster inclusion by assisting with travel, activities, and community participation, actively removing barriers (e.g., accessibility, communication) to enable full involvement.

  • Build relationships by encouraging natural friendships and trust, complementing—not replacing—family and peer connections.

  • Collaborate with people with lived experience to co-design support plans, ensuring their voices shape services.

  • Participate in team meetings, update risk assessments, and follow safeguarding, accident, complaint, and incident procedures to uphold Human Rights and safe practice.

  • Maintain familiarity with support plans and records; liaise with individuals, their chosen contacts, and colleagues to report changes or concerns transparently and efficiently.

  • Update electronic records accurately, ensuring compliance with data protection and health/safety policies, reflecting Integrity.

  • Work flexibly to meet service needs, adapting to promote a compassionate, inclusive culture.

Customer Service

  • Handle queries from people we support, colleagues, and partners professionally, with empathy and efficiency.

  • Maintain accurate, timely records to ensure transparency and high-quality service delivery.

General Duties

  • Undertake additional tasks as directed by ACT management, including supporting community partnerships and representing ACT at meetings to advance Inclusion.

  • Contribute to developing inclusive practices, sharing honest feedback to align with our vision and values.

  • Participate in regular performance reviews to ensure continuous improvement and integrity.

  • Perform other duties appropriate to the role, always acting with courage to do the right thing.

Person Specification

​

Essential Requirements

  • Excellent communication skills, adaptable to diverse needs, including accessibility challenges.

  • Experience of supporting people who may have behaviour of distress is essential.

  • Positive values toward disabled people, equality, and inclusion, with proven ability to uphold Human Rights and Relationships.

  • A curious, reflective approach, valuing lived experience and co-production ( to empower person-led outcomes.

  • Friendly, approachable manner with strong teamwork and problem-solving skills to build trust and Inclusion.

  • Good working knowledge of IT packages (e.g., for record-keeping) and key legislation (e.g., Mental Capacity Act, Equality Act) to ensure compliance.

  • Willingness to learn, develop skills, and engage in training on safeguarding, equality, and barrier removal, provided by ACT.

  • Full UK driving licence and access to a vehicle is required as the role may involve transporting people we support.

 

Desirable Requirements

  • Experience in adult social care or supporting disabled people in community settings, ideally with co-production involvement.

  • Knowledge of overcoming barriers to community belonging.
